Data Access Request Form

The Consortium maintains a database composed more than 1200 datasets. This database was compiled as a multi-site effort across three years. To request access to data from this database, please download the form below. The completed form - which highlights your proposed project, the data you wish to access, and the publication plan - should be emailed to austin.alley@cchmc.org and will be subsequently reviewed by the Consortium’s database sub-committee. The investigator will be invited to deliver a brief 10-minute presentation on the proposed project in the sub-committee’s next meeting.
